I believe that the unit of care is first the patient then the family. The patient is the reason of the visit, so they need to be the main priority. If the patient passes away, the family will then be a huge priority because some may not take it very well and may need help such as a counselor or they may fall into depression. But in all, the patient is the unit of care because they are the one who is having a problem that needs to be taken care of ASAP. I think that family means the people who care and love you. The people you are closest to. My family considers my boyfriend part of the family because they all love him like they do a blood relative.
I agree with Stacey that the unit of care is both the patient and family. Though the human being which is dying is the main concern, the family must also learn to deal with the inevitable loss of that person. Both people are affected deeply by the terminal illness and must find ways to deal with the emotional and physical trials that they must endure. Family can mean immediate family (brothers, sisters, mothers, children, husbands, wives), extended family (aunts, uncles, cousins), and friends. Basically, anyone who is suffering because of the patients illness.

The situation as a whole is experienced by the patient and family, just different ways. The patient experiences the hands-on care and the family has to deal with their thoughts and feelings about the situation. In a family, for the most part, if a family member is in the hospital it effects the rest of the family in different ways.
The patient is definitely the primary unit of care. If I was a physician I would not make a choice to benefit the family that would ultimately harm the patient. However, it is important to keep the family in mind when treating the patient.
I would make a distinction between family and relations. In terms of relations, yes they are close to you genetically, however are they close to you emotionally. I would say family is anyone who is close to you emotionally, whether or not they are closely related to you.

If the patient can't comprehend or is terminally ill then the family needs to be kept in mind. Most of the time the family knows what the patient would want. Family is those that are close to you and people that you trust. Most of the time it could be their children or even your parents.
The unit of care has to be the primarily focused on the patient. However, I do think that its important that there can be some sort of comfort for the family of the patient. For some this is maybe the first death or serious sickness they have experienced and comfort for them is extremely important. But, the patient is the one who is sick therefore to one second not think that they should be the primary focus is rather absurd. Family to me means everything without it I can say that I would probably be lost and not who I am today. It is those who love and care about me most and will always be there for me.
